India’s trusted workhorse in space exploration, the Polar Satellite Launch Vehicle (PSLV), witnessed a rare failure during ISRO’s 101st satellite launch. The Indian Space Research Organisation (ISRO) confirmed that the PSLV failed to place the satellite in its intended orbit, marking only the third such failure in its otherwise stellar record.
ISRO scientists closely monitored the launch sequence from the Satish Dhawan Space Centre in Sriharikota. The PSLV lifted off on schedule, but a technical glitch in the fourth stage disrupted the satellite’s path. Officials later clarified that the onboard navigation system malfunctioned, causing the deviation.

A Look Back at PSLV’s Previous Failures
The PSLV has successfully completed over 95 missions since its inception, earning global acclaim for its reliability. However, this latest incident joins just two other failures in its history. The first occurred in 1993 during PSLV’s maiden flight, when a control failure caused the vehicle to spiral out of control shortly after take-off.
The second failure happened in 2017 when the PSLV-C39 mission ended unsuccessfully. Engineers traced the issue to a heat shield that failed to separate, leaving the satellite trapped inside. Despite these incidents, PSLV remains ISRO’s most reliable launcher, having deployed satellites for more than 30 countries.
ISRO chief S. Somanath reassured the public that the team has already initiated a detailed failure analysis. “We will investigate every aspect of the malfunction and rectify the issue for future launches,” he stated during a press briefing.
India’s space program, while momentarily shaken, continues to hold global respect for its cost-effective missions and technological advancements. Experts believe this minor setback will only strengthen ISRO’s resolve to innovate further.
